چکیده

PURPOSE The authors observed the unusual complication of inverse Bell's phenomenon of two cases following repeated and extensive levator resection surgery for a blepharoptosis and suggest the possible mechanism of the phenomenon. METHODS Case report. RESULTS The first case was a 22-year-old woman who was diagnosed with a congenital ptosis with good levator function and she underwent levator resection and several reoperations because of asymmetry of lid height. Inverse Bell's phenomenon and resultant exposure keratitis were found in both eyes following the final surgery and they resolved in 1 month. The second case was a 19-year-old man with a right residual congenital ptosis. After a maximal levator resection was performed, he had a satisfactory lid height. Inverse Bell's phenomenon of right eye developed and reverted to normal after 2 weeks. The injury of oculomotor nerves in superior rectus during the repeated and excessive operation would alter a trigemino-oculomotor projection but it is unlikely because the phenomenon resolved without any treatment. Severe edema and hyperemia of the superior fornix following the operation may aggravate the relationship between eyelid and superior rectus movements. CONCLUSIONS The simultaneous resolution of eyelid edema and inverse Bell's phenomenon supports our hypothesis that the latter may be determined by the edema of the soft tissue secondary to the surgical manipulations rather than the aberrant connections of the nervous system. When the inverse Bell's phenomenon develops postoperatively, copious use of lubricant and close follow-up of corneal complication is required until it resolves.

Inverse Bell's Phenomenon: Rare Ophthalmic Finding Following Ptosis Surgery.

Bell's phenomenon is protective reflex in which the globe is turned upwards and slightly outwards during the eyelid closure to avoid corneal exposure. In Inverse Bell's phenomenon, the eye moves downward instead of upward, this may be seen in the normal population, patients with Bell's palsy or following conjunctival scarring. We hereby present the unusual complication of transient inversion of...

Levator Resection in the Management of Myopathic Ptosis

PURPOSE To evaluate the results of levator resection in patients with myopathic ptosis. METHODS The medical records of consecutive patients who underwent levator resection surgery performed for myopathic ptosis between October 2009 and March 2013 were reviewed. Indications for surgery were ptosis obscuring the visual axis and margin-reflex distance ≤ 2 mm. Surgical success was defined as clea...

The Relationship of Amount of Resection and Time for Recovery of Bell’s Phenomenon after Levator Resection in Congenital Ptosis

BACKGROUND Recovery of Bell`s phenomenon after levator resection is unpredicatable. Delayed recovery can result in vision threatening corneal complications. AIM To study the variability of Bell's phenomenon and time taken for its recovery following levator resection for blepharoptosis and to correlate it with the amount of resection. METHODS A prospective observational study was conducted o...

Management of ipsilateral ptosis with hypotropia.

Thirty-one patients presented for surgical correction of unilateral hypotropia of the globe and blepharoptosis. The hypotropia and pseudoptosis were corrected by Knapp procedures. The Bell's phenomenon was thereby improved, allowing safe correction of the true ptosis, generally by an anterior levator resection whose magnitude depended on measured levator function.

Transcutaneous Blepharoptosis Surgery: Simultaneous Advancement of the Levator Aponeurosis and Müller’s Muscle (Levator Resection)

Transcutaneous blepharoptosis surgery with simultaneous advancement of the levator aponeurosis and Müller's muscle (levator resection) is a popular surgery which is considered effective for all types of blepharoptosis except for the myogenic type. Repair of ptosis cases with good levator function yields excellent results. A good outcome can be also obtained in cases with poor levator function, ...
